Qualifications:

Family Nurse Practitioner in a large multidisciplinary primary care clinic. I provided direct patient care in the adult and family medicine department with an average of 15-20 patients per day. I maintained a panel size of approximately 1500. I independently assessed, diagnosed and treated a variety of acute and chronic conditions with full prescriptive authority. I performed minor procedures and conducted male and female physical exams.  
Team 5000 member: A select group of physicians, nurse practitioners, nurses, medical   assistants, and administrators with a goal of creating a medical home team and improving patient outcomes, patient satisfaction, access to care and creating a combined panel size of 5000 patients per team.
