Educate actively managed patients about chronic conditions, healthy behaviors and strategies. Provide assessment/management of Diabetic Clinic and utilize treatment guidelines to care for patients. Full-time position with great benefits. Requirements include Current unrestricted State licensure to practice in California. Current Drug Enforcement Agency (DEA) licensure.
