RE: LETTER OF AGREEMENT

Recruitment of Physician Assistant or Nurse Practitioner

Dear Employer:

It is agreed that in the event our introduction and presentation to you of a qualified Physician Assistant or Nurse Practitioner candidate results in an offer of employment that is accepted by the individual, then the following fee, terms, and conditions are applicable.

1. PROFESSIONAL FEE

A. Our fee, to be paid by Employer, assuming the presented candidate is selected for employment by you, is $10,000. This fee will be invoiced only upon an offer being made by you and accepted by the candidate.

B. The fee is payable by you 30 days AFTER the candidate's employment start date. 

2. GUARANTEE

If the subject candidate is selected by you for employment through our efforts, his/her association with you is guaranteed by us in the following manner:

We guarantee the performance of the candidate for a period of 90 days in the position for which he/she has been selected. Accordingly, we shall provide a replacement, but only for the identical position, at no additional fee if performance is unsatisfactory and if selected candidate is terminated or leaves your employment. Under no circumstances will there be a refund of fees nor a credit towards future engagements if you elect not to replace the selected candidate with an individual for the identical position.

3. FUTURE PLACEMENT

If the candidate named above is recruited by you for any position within "Company" or its affiliates, for a period of 18 months subsequent to our initial introduction, we shall have earned a fee identical to the conditions as stated above.

4. AGREEMENT ACCEPTANCE

Your acceptance of the referral of the candidate constitutes your agreement with our terms and conditions as stated above. Our candidate may not be referred to any entity outside your own organization without assuming financial responsibility.
